     Definitely a good idea for better patient interaction, however one must keep in mind that the form isnt very long to fill as also not too many questions or not complicated

    It must be as simple as possible and not consume too much of the patients time

     I feel the feedback forms must be kept in the waitng area and not given to the patients voluntarily. If they feel the need to suggest some measures to improve the services rendered by you, they can pick up a form themselves and fill it.

     I agree with the above idea because giving a feedback form to the patient would seem a little too desperate on the part of the doctor. And besides the patient may feel forced to fill the form.
