I encountered a case where a female aged 35 years had deeply carious tooth with periapical infection in 36.
RCO was done, after few days BMP was completed.

After last sitting where a Ca hydroxide dressing in canals was given, patient got swelling in that area externally and on examination we saw a boil [similar to a sinus opening] with no sign of pus in area of buccal gingiva of 36.
I & D was done, open dressing given, antibiotics prescribed and patient recalled after few days
Kindly opine if there is any other protocol to be followed, also what could be the probable cause of this swelling, even though Ca hydroxide dressing was given and there waqs no swelling prior.

* Start her on antibiotics and NSAIDs for 7 days
* Check dryness of canals.
* If there is too much of infection then you can irrigate the canals ( and the periapex) with injection gentamycin. I do it in very rare cases
* Drain the abscess.
Most of the times a proper biomech supported by antibiotic cover does the job. RCF should wait till the lesion heals up.
Post IOPA if possible.
Hope this gives you some ideas.
regards,
veeren

we put her on NSAIDs and antibiotics, canal was dressed in ca hydroxide..today the patient came, swelling reduced, but pain is still there and that sinus like opening is gone
So that s a good sign…now we have put IODOFORM in the canals
Il put the IOPA soon… there is slight infection in the furcation area
