A common complication following extraction of teeth is dry socket formation. It occurs as a result of dislodgement of the blood-clot from the extraction socket due to early removal of the pom-pom, or inadequate pressure on cotton held in extracted area, forceful coughing or sneezing and most importantly, spitting out..All this leads to dry socket formation.

A typical dry socket shows exposed bone and patient gives symptoms of tremendous pain despite taking pain-killers.

    TREATMENT:
First of all, a thorough irrigation of the extraction socket needs to be done with betadene solution.
Once the area has been disinfected, a ZOE pack should be made.
This pack is similar to the one dispatched in restorative dentistry as a temporary pack, the only difference being, small cut up pieces/bits of clean, autoclaved gauze have to be incorporated.
This ensures that the pack stays in place.
The pack is placed in the socket and patient is recalled after a span of 5-7 days.
Also the patient is asked to continue analgesics and low dose antibiotics.

    alveogyl may work sometimes.

alternatively you can curette the socket under LA, cause bleeding and than suture it up.

If you are extracting a very badly infected tooth or if the extraction has been traumatic if you suspect that a dry socket might occur than it would be better to suture the extraction wound.

If the extraction is traumatic one can also place ‘abgel’ inside the socket before suturing to control bleeding..

abgel is required to be placed when there is bleeding. there need not be bleeding in a difficult extraction.

    HOME REMEDIES

Clove oil has proved to be one of the best remedies for dry socket. Firstly, dip the cotton in water. This, will reduce the sting of clove. Then, dip it in clove oil and dab it on the socket. If the teeth around it are paining, then you can dab this cotton swab on them too. This dry socket treatment at home can be done once or twice a day.
Another dry socket home cure is to gargle your mouth with lukewarm water and salt.
Make it a point to add yogurt to your diet as it works as a natural antibiotic.
Most of the times, dry socket leads to swelling. To reduce the swelling, you can use an ice pack.
Another option is to use a cold tea bag and press it with the help of your teeth on the area where the pain is intense. The tanic acid present inside the tea bag relieves the pain.
In a cup of lukewarm water, add a pinch of turmeric and rinse your mouth with this solution.
Prepare a mixture of dry salt, turmeric and mustard oil. Apply this mixture on the dry socket. This mixture will surely alleviate the pain along with maintaining proper dental health.
People consuming tobacco in any form should avoid its intake for at least a week to relieve the pain.
You can bite a moist piece of gauze. Make sure that you change the gauze whenever required. This dry socket home cure will avoid the saliva and air reaching the dry socket. Saliva and air are known to intensify the pain.
Wash your mouth with peroxide solution. But while doing this, take care that you do not swallow it. This home remedy for dry socket reduces the pain immediately.
Another option is to hold cold packs from the outside. This will decrease swelling and pain thus providing dry socket relief.

What does alveogyl contain and how is its action/content different from abgel?

i do not exactly know what it contains but it acts on the nerve endings and sort of anethtsizes them and thus provides relief.

abgel is to protect and stabilize the blood clot after extraction.

abgel should not be used in diabetics. most of the time if you suture the extraction wound properly than there is no need for abgel.
