we have placed an two piece anterior metal ceramic bridge to a 25 yr old male

we have placed an two piece anterior metal ceramic bridge to a 25 yr old male patient ….two days after the insertion ceramic chipped off from the mesial aspect of left anterior incisor region . There are no lateral forces excerted as the bridge is not in contact with the lower incisors.what could be the possible clinical and laboratory causes for its failure .
