A feedback form would be a good way of interacting with patients and maintaining relations however one cant force them to do so since many patients may find it overpowering or tedious
a better option instead of feedback forms would be a SUGGESTION BOX where a box with a scribble pad is kept and the patient is free to write down his comments whenever he feels to do so
