    A patient 50/M needs a root canal to be done with upper right 2nd molar but the patient has limited mouth opening, difficult to have an access opening even with the use of short shaft bur..can anybody guide me how to go about with the case?

    what is the cause of limited mouth opening ? can it be cured in the near future ?

    There are various causes of limited mouth opening like,
    OSMF,
    Tetanus,
    Post radiation therapy, Systemic Sclerosis (Scleroderma)
    Ankylosis,
    Microstomia,
    Post Surgical Fibrosis,
    Pericoronitis,
    Injury to Medial Pterygoid muscle. etc

    exactly so if the restricted mouth opening can be treated soon it would be prudent to provide symptomatic relief and wait for the mouth to open,.

    otherwise doing RCT would be difficult

    put him on antibiotic course,,including muscle relaxant,,,,

    I think we can do RCT by any laser technique….And there are many excerise therapy for post treatment …

    hello.

    There is no cause for limited mouth opening.The patient can naturally open only limited mouth.Kindly guide if anything can be done.

    In my view treatment should be postponed until mouth opening is sufficient..prescribe antibiotics and analgesic until the infection subsides and ask him to take analgesic whenever there is pain..meanwhile investigate the cause for limited mouth opening and treat it accordingly..

    And in case if the mouth opening cannot be treated in the near future refer the patient to an oral surgeon for extraction.

    See if you can hold the files and reamers with a small artery forceps and than perforem biomech for endo.
