    plz tell the treatment of generalized severe attrision with upper and lower tooth in a 55year old female patient with fixed appliance.

    pt is having gen sensitivity, with rct done with upper and lower ant teeth becoz of exposure,was also concern abt her aesthetics as affected by decreased vertical dimension of teeth. She wants to prevent the further damage of her teeth and their is no associated tmj problem.

    Assuming that the periodontal condition is normal what we need to find out first is to how much tooth structure needs to be built up. If there has been a lot of destruction of tooth structure than you may need to use posts and cores to provide stability to the crowns.
    To increase the bite you may need to place crowns on all teeth. First place temporary crowns for at least six weeks and see if the patient is comfortable. Then convert them into permanent crowns.
    If there has been a lot of attrition it would be preferable to perform endodontic treatment on all teeth which need to be crowned.
    The case will need to be planned on a hanau articulator.

    one more thing………1..how to get vertical diamention at rest (vdr) and centric relation of the dentulous patient as in these case without using facebow?

    But is doing RC on almost all teeth followed by crowns a feasible and practical option considering the cost, time and effort involved in it? Cant we simply give a complete overdenture considering that the patient is 55 yrs old? This option helps us save on all the 3 parameters mentioned above and the vertical height can also be restored.

    the youth now is between 20 year to 60 years . 60 years to 80 years is middle age.

    From the initial description overdenture does not seem to be a feasible option. Proper case selection and counselling is a must.

    What are the contraindications for overdenture in the above case besides the age?
