(Extra Oral Endodontics: A new concept in the treatment of Rare Dental Diseases & Developmental Anomalies)
Elective Replantation is not a new entity for Dental Surgeons. My Elective Replantation Technique is a modified one because I prefer Extra Oral Endodontics. Extra Oral Endodontics has an edge over Conventional Endodontics (especially in multi-rooted teeth), which requires multiple sittings and sophisticated equipment & instruments along with Specialised training in Endodontics.
My E.O.E. Technique can also be applied to treat teeth having rare pathological conditions or developmental anomalies or anatomical limitations for apicectomy, with highly satisfactory results. Here, I have used this technique successfully in a case of Internal Resorption of Mandibular Second Molar leading to Radicular Cyst and Extra Oral Sinus on left cheek.
The discharge from sinus stopped and extra oral wound started healing within three days. The replanted tooth got well attached with surrounding bone and it became firm and fully functional. STUDY DESIGN: Extraoral root canal treatment was performed from a retrograde direction with posts made of ceramics or titanium. Preoperatively, the pulp status was classified as definitely infected (n = 47) or not infected/symptomless (n = 78), and the condition of the periodontal ligament (PDL) was classified as damaged (n = 50) or not damaged (n = 75) according to trauma type and extraoral storage. The healing after replantation or transplantation was diagnosed as functional, ankylosis, or infection depending on clinical and radiographic findings.
RESULTS: In total, 125 teeth in 99 patients were replanted or transplanted. Mean observation period was 44.4 months. Teeth with a less damaged PDL in no case exhibited ankylosis/replacement resorption after a mean observation period of 53 months. Teeth without preoperative infection of the pulp in no case showed infection-related complications after a mean observation period of 30 months. Teeth classified as definitely infected before treatment had a high frequency of continuous periradicular bone and root resorption.
CONCLUSIONS: The extraoral insertion of posts appeared to inflict no additional damage to the PDL that was clinically relevant. Infection-related complications were prevented. Teeth that were classified as definitely infected at the time of treatment had a low rate of healing and should undergo conventional root canal disinfection before this surgical procedure is applied.

2)What is preferred while splinting in terms of strength and better results—-composite splints or stainless steel?
3)Where can one read more on this topic..any particular book or journal..in terms of studying it theorotically?
4)Splinting methods?
5)Since the procedure is a delicate one a high level of disinfection needs to be followed…could you please elaborate on it? As in what extra care needs to be taken while handling the tooth extra-orally?

I have my (records in detail) my PC and wish that someone takes interest in analysing them. Data are available with Pre and post and follow up X-rays of all subjects. The results are highly encouraging. I had presented papers in 3rd. Commonwealth Congress at Delhi in the year 2000 and at FDI in 2004.
