Sometimes a tooth cannot be recovered by means of root canal therapy. This may happen:
When a big caries appears that damages a considerable part of the dental structure, impeding an efficient tooth reconstruction
When the periodontal support is weak
When the tooth’s crown and/or its root has been fractured
Should these clinical events appear, the alternatives to an endodontic treatment are:
Tooth extraction without replacement: it is the less desirable clinical option. If extracted teeth are not replaced, adjacent teeth will move towards the holes. That will lead to mastication problems, pathologies due to inclined teeth and biting collapse; affecting all these events the facial aesthetics, and being responsible for temporo-mandibular articulation pains. Finally, the future reconstruction of the denture will be more complex and more expensive.
Tooth extraction with replacement by a fix prosthesis (fix bridge): this option entails a traditional replacement. In our opinion it is more suitable when adjacent teeth (“pillars”) are affected by caries or fillings.
Tooth extraction with replacement by a dental implant: Dental implants have two parts.:
A special metallic root that is introduced into the bone at the same place where the original natural root was situated (osseointegration). The osseointegration is a biological procedure that may take 6 to 12 months.
A clinical prosthetic crown similar to the original tooth crown.
Currently, the therapy by means of dental implants offer excellent results.
