    There is a patient who has got two screw retained implant crowns made for lower left 1st and 2nd molar.

    The screw is tight but still both the implant crowns ( porecelain fused to metal with some metal island on the occlusal surface as the interocclusal space was less)  are slightly mobile.

    Whst could be the possible causes ?

    Hope to see a lot of responses and many thanks in advance for the same.

    What I understand from the case posted sir, if it’s possible to post some xrays, clinical photographs, working casts etc could help more in understanding the situation.

    Coming with the available information, whenever screw is tight but still crown is mobile, according to me, load is transmitted not onto screw but to the implant abutment connection due to OCCLUSAL OVERLOAD (as metal island is there due to less INTEROCCLUSAL space).

    This could lead to Implant Fracture…

    One more important thing to be noticed is also how tight are the contact areas, as that also may influence this.

    Be cautious about this and if possible correct the Occlusion and, if not adjustable, may have to repeat the Prosthetics for long term peaceful sleep and long term success of prosthesis.

    Thank you Dr. Sorabh for the reply. This w.as a screw reatined implant crown

    I had a meeting with my dental technician who had fabricated the prosthsis. According to him the slot of the crown was wider.

    Therefore, he suggested to remove the screw and refix it with a plumbers tape on the screw head. I have done that yesterday and for now the crown is not mobile.

    Keeping fingers crossed altho…

    The other options were sandblasting the screw slot with some ceramic powder etc.
